Which direction should I choose?

“90° right” turns pages clockwise, “90° left” counter-clockwise, and “180°” fixes upside-down scans. All pages rotate together.

Will rotating affect quality?

No. Rotation only updates each page's orientation metadata — the content isn't re-rendered or recompressed, so quality is untouched.
